To register your Family Membership fill out the registration form below. Please be sure to fill out ALL the lines and sign everything. Once we have your form and background check complete, we will:

  • contact you with a login to the Parent Portal

  • send Important Documents for your e-signature

  • invite you to our Private Groups on the Band App platform for Program communications and other details!

We are a VOLUNTEER organization, so please be patient as we process your Family Membership Registration and get back with you!
